SIRI and COSIRI specific Terms & Conditions

When booking a SIRI or COSIRI Certified Assessor course, the following specific Terms and Conditions in addition to those covered under "General Terms & Conditions" below.

1. Certification Scheme:

1.1

MidasDX.com is an appointed Training & Examination Body administering SIRI & COSIRI Assessor Certification Schemes on behalf of the International Centre for Industrial Transformation (INCIT).

1.2

The SIRI & COSIRI Assessor Certification Schemes aim to safeguard the integrity and consistency of the Official Assessments conducted by ensuring the quality and proficiency of the CSA/CCA.

1.3

The SIRI & COSIRI Assessor Certification Schemes consists of 2 parts:

1.3.1

The 5 day (40 hour) online, instructor led SIRI/COSIRI Assessor Course (please see the ‘SIRI/COSIRI Assessor Training Course Booklet’ for more details).

1.3.2

The CSA/CCA Examinations, which consist of two sections: a theory paper comprising multiple-choice questions (MCQ) and a practical case interview.

1.4

The course can be taken separately, however the title of Certified SIRI/COSIRI Assessor, access to the SIRI/COSIRI Portal, and the authorisation to carry out SIRI/COSIRI Assessments will only be granted upon successful completion of the CSA/CCA examinations.

1.5

The 5 Day SIRI/COSIRI Training Course must be attended in full to be eligible to sit the CCA/CSA examinations.

1.6

It is recommended that the exams be taken no earlier than 2 weeks after completing the course to ensure sufficient reflection and revision of the course materials has taken place, and no longer than 8 weeks after completing the course to ensure that the material is still fresh in the participant’s mind.

2. Pre-requisites:

2.1

Previous experience requirements (SIRI/COSIRI):

2.1.1

SIRI – In addition to attending the 5 Day SIRI Assessor Training Course, participants must meet certain pre-requisites to register for the CSA Examinations. Please refer to Section 3 of the ‘SIRI Factsheet July 2023’ for details of the pre-requisites that must be fulfilled. You can find the document here: SIRI Factsheet.pdf. All applicants are required to possess either academic qualifications, or professional experience, or both.

2.1.2

COSIRI - In addition to attending the 5 Day COSIRI Assessor Training Course, candidates must meet pre-requisites, such as having relevant manufacturing and consulting work experience, to be eligible to register for the Certified COSIRI Assessor Examinations. 2.2

Evidence of relevant manufacturing and consulting work experience will need to be provided prior to participants being registered for the exams. An online form will be provided by MidasDX.com for participants to submit their evidence.

2.3

By paying for the SIRI/COSIRI 5 Day Training Course and/or Examinations, you acknowledge that you have read, understood, and agree to the Terms and Conditions set forth in this document.

General Terms & Conditions

3. Code of Conduct:

3.1

Participants are expected to adhere to a professional code of conduct during the course. Disruptive or inappropriate behaviour may result in removal from the course without refund.

4. Invoicing and Payment:

4.1

Invoicing will be issued upon receipt of confirmation from the client. Invoices are payable in the currency stated on the invoice. Exchange rate fluctuations and related costs (if applicable) are the responsibility of the purchaser.

4.2

The fees for the course and, where applicable, the associated examination will be invoiced together. However, you may choose to pay them separately by first settling the course invoice, followed by a second payment for the examinations when you are ready to take them.

4.3

For coach-led training courses, payment in full for the course must be received at least 14 days prior to the commencement of the course. All funds must be cleared prior to any further information or course material being issued. Failure to meet this payment schedule could result in the place reserved for the participant being released for others to book.

4.4

For instant-access, online courses, payment must be cleared before access to the course is granted.

4.5

Payment in full for the examinations must be received prior to participants being registered for them.

4.6

Payment and banking fees are to be borne by the payee. Please use the payment type OUR when paying. This ensures that, for international transactions, transfer fees are correctly allocated.

5. Remuneration and Taxes:

5.1

MidasDX.com will receive remuneration for services as per agreed prices. Prices do not include any applicable VAT, withholding tax, duties, or other governmental charges. The client shall bear these additional costs as required by law.

6. Cancellation Policy:

6.1

For coach-led course cancellations, the following charges will apply:

More than 14 days before the event: 100% refund applicable
Between 7 and 14 days before the event: 50% of course fees will be charged
Less than 7 days before the event or during the event: 100% of course fees will be charged

6.2

Exceptions to the cancellation policy may be considered on a case-by-case basis with valid documentation provided. All relevant fees to process the refund will be borne by the participant.

6.3

Should there be a need for MidasDX to cancel a course, participants will be offered a place on an alternative course of the same type. In the event where this cannot be accommodated, a full refund will be issued.

7. Minimum Numbers:

7.1

A minimum number of participants is required for coach-led courses to proceed. MidasDX.com reserves the right to cancel the course if the minimum number is not met. Should this occur, as much notice as possible will be given, but not less than 1 week prior to the course start date.

8. Feedback and Evaluation:

8.1

Evaluation forms will be provided for participants to fill out at the end of the course.

10.1

The content of this proposal is the property of MidasDX.com and should be treated as confidential. It should not be shared with anyone outside the client's organisation without written consent from MidasDX.com.

11. Intellectual Property Rights:

11.1

All course materials, including but not limited to training content, presentations, and documentation, are the exclusive property of MidasDX.com. Any unauthorised use, reproduction, distribution, or modification of these materials is strictly prohibited.

12. Liability and Disclaimer:

12.1

While every effort has been made to ensure the accuracy and relevance of course content, MidasDX.com does not accept liability for any errors or omissions. Nor shall we be liable for any loss or damage arising from the application of the information provided in this course.

12.2

To the fullest extent permitted by law, MidasDX.com shall not be liable for any direct, indirect, incidental, consequential, special, or punitive damages, or any loss of profits, revenue, data, or use, incurred by you or any third party, whether in an action in contract, negligence, breach of statutory duty, or otherwise, arising out of or in connection with your participation in the course or use of course materials.

12.3

MidasDX.com's total liability to you for any claims arising out of or in connection with the course, whether in contract, including negligence, breach of statutory duty, or otherwise, shall be limited to the amount of fees paid by you for the course.

12.4

MidasDX.com shall not be liable for any failure or delay in performance due to circumstances beyond its reasonable control, including but not limited to acts of God, war, terrorism, strikes, pandemics, or interruptions in communication or internet services.

12.5

The above limitations and exclusions apply to the maximum extent permitted by applicable law. Some jurisdictions do not allow the exclusion or limitation of liability for certain damages, so some of the above limitations may not apply to you. In such cases, MidasDX.com's liability will be limited to the fullest extent permitted by applicable law.

13. Indemnification:

13.1

You agree to indemnify, defend, and hold harmless MidasDX.com, its affiliates, officers, directors, employees, agents, and licensors from and against any and all claims, damages, losses, liabilities, costs, and expenses (including reasonable attorneys' fees) arising out of or in connection with your participation in the course, your breach of these terms and conditions, or your use of any course materials.

13.2

This indemnification obligation will survive the termination or expiration of these terms and conditions and your participation in the course.

14. Data Protection and Privacy:

14.1

We are committed to protecting your privacy and personal data. All personal information collected during the course enrolment and execution will be processed in accordance with our Privacy Policy, adhering to GDPR.

15. Dispute Resolution:

15.1

Any disputes arising in connection with this course will first be attempted to be resolved through mutual negotiation. If unresolved, disputes shall be submitted to and finally resolved by arbitration under the laws of the United Kingdom.
